I rescued this boa at first I thought her head was just a little swollen (she was in full blue) I finally got the top of her head shed unstuck and found this… any idea what could have caused that

If it is swollen the Boa needs to see a vet, reguardless of what caused it.
It could be a rodent bite, but there’s no real way to tell without asking the previous owner…
The top of the wound between the nostrils looks like it is wet and oozing and if her shed was stuck to it, it may mean the wound happened fairly recently and the chance of infection is really high. If you have betadine on hand you can gently clean that wound with 1:10 parts betadine:water and a q-tip to keep the area disinfected 1-2x a day until you can get to a veterinarian.
To me this looks like a pushing injury. The snout and scar over the eye could be prey related but the straight scratch on the right side behind the eye makes me think she tried to push her head through an opening that was too small.
No matter the cause, keep an eye out for infection.
